What the law asks for
In the US and many other countries there is no mandatory psychosocial assessment: ISO 45003 is the voluntary reference. Personality tests should never be used to screen candidates: in the US that can run into the ADA, Title VII and local laws such as NYC Local Law 144.

FAQ
Does the questionnaire replace the company's risk management?
No. It feeds your risk inventory and action plan, which remain the responsibility of the company and its health and safety team.
Do employees have to identify themselves?
No. The assessment is anonymous and wellbeing tests are visible only to each person.
